When a cancer warning isn’t the final diagnosis

One person’s diagnostic journey began with a primary care doctor identifying a benign skin condition. A dermatologist later said that, because of its rapid onset, it must be cancer and that the person needed to see their doctor immediately for cancer testing. What was missing was the reasoning behind that conclusion. The rare presentation was typically associated with three possibilities. Two did not appear to fit, leaving cancer as the third. A biopsy confirmed the skin condition. A rigorous diagnostic workup ultimately confirmed that the person did not have cancer. The final finding was a rare form of the condition whose cause remained unknown.

Looking back, the person wished that distinction had been explained from the beginning: confirming a condition and investigating its possible cause were different parts of the diagnostic process. An explanation of the three possibilities—and why two did not fit—would have made the concern about cancer easier to understand. So would a clear explanation of what the biopsy could establish and what further testing was needed to determine. This experience highlights the value of understanding not only what clinicians suspect, but why they suspect it, what remains uncertain, and which tests can answer which questions.
